Congressional Medal of Honor - Air Force

Two Congressional Medal of Honor plates may be issued for a car or light truck owned by a recipient of the Medal of Honor. No annual fee is collected for the first set of plates. Additional sets are free, but you must pay your regular registration fee and other applicable fees. The plates are mailed directly to the applicant. The surviving spouse of the Medal of Honor recipient can continue to register one vehicle with these plates as long as the spouse remains unmarried.

A written request with a copy of the applicant's U.S. Department of Defense Form DD-214 or a copy of the award certificate must be submitted to:
