What should I do if I cannot open or view an online form?
Many of our online forms are in a PDF format that can only be viewed or printed with Adobe Acrobat Reader 7.0 or higher. If you don't have Adobe
Acrobat Reader or are using an older version, you won't be able to open the forms. Install or upgrade to a newer version; you can
download it free. It is important that you first
uninstall any prior versions of the Reader before downloading and installing the latest version.
You also could be experiencing problems for other reasons:
- Your Internet Service Provider (ISP) might be experiencing heavy traffic.
- The file size could be too large for your Internet connection speed. Read the FAQ on how to download a form directly to your computer's hard drive.

What should I do if the form looks like it has loaded but does not open?
If this happens, refresh your Web browser. If that fails, make sure you have Adobe Acrobat Reader 7.0 or higher installed properly. If you do not have Adobe
Acrobat Reader, you can download it free.

How can I download a form directly to my hard drive and stop the form from displaying in the Web
browser window?
Use the "Save Target As..." option to download the PDF file directly to your hard drive:
- Go to the page containing the link of the form you are trying to save.
- Move your mouse pointer over the link on the page.
- Right click (or left click if your mouse is configured for left-handed operation) on the link.
- Click on "Save Target As..." when presented with a menu.
- Select a location on your PC to save the file.
Rename the file when saving the form. Unless you create a new file name, all forms saved using this method will receive the name: "GetForm." This is due to
the special Web link used to allow access to forms within our Electronic Forms Management System.
Use Adobe Acrobat Reader to open the file after downloading. If you do not have Adobe Acrobat Reader, you can download it free.

When downloading a PDF form, my Web browser window says, "Done," but all I get is a blank
screen. I already have the latest version of Adobe Acrobat Reader. What should I do?
If a PDF file appears as a blank window within your web browser after downloading, follow these steps to resolve the issue:
- Click your browser's "Refresh" button.
- Verify that you have the correct version of Adobe Acrobat Reader. If you are using a version prior to 7.0, upgrade to version 7.0 or later. You can download it free.
- Try using the "Save Target As..." (or "Save Link As...") option to download the PDF file directly to your hard drive and bypass the Acrobat Reader plug-in. Read the FAQ on how to download a form to your hard drive.

When I download a PDF form, I get a message that the PDF file is damaged or corrupted. Is your file damaged?
All of our PDF files are verified prior to posting on the Web site. It's likely your download was not successful. Exit your browser and try to download the file
again. If you are still experiencing problems, please try possible solutions addressed by Questions 1-4 before calling forms system support at (866) 560-4755.

When I tried to download a PDF form, I got a message asking me what application to use. I selected
Word (or another program). Now all I see are weird symbols when I look at the file. What can I do?
The easiest way to fix your problem, is to install Adobe Acrobat Reader. Once the installation is complete, your
computer will no longer use Word to view the PDF file. Your computer will use Adobe Acrobat Reader to view the files instead.

How can I save data I've put in the PDF forms?
Adobe Acrobat Reader will not allow you to save the form data. You can only save a copy of the blank form. If you need a copy of your data, print a copy of the completed form before closing it.

When printing some forms in PDF format all the words run together with no spaces between them;
however, everything looks fine on the screen. What is causing this?
There are a variety of reasons that could cause a PDF file not to print properly. Please see Adobe's Support Knowledgebase on
troubleshooting printing problems. This issue can often be resolved by
obtaining and installing the most recent print driver available for your particular make and model printer. Most printer manufacturers provide free
printer driver updates from their Web sites.

How do I find out which version of Adobe Acrobat Reader I use?
Locate the Acrobat Reader program on your machine and open the program (you probably have an icon on your desktop for
Acrobat Reader that you can double-click). Click on the "Help" menu item at the top of the Acrobat Reader window.
Select "About Acrobat Reader" from the menu options. A window will appear that displays the version.
